MediZoom Transportation, Terms & Conditions

SERVICE AGREEMENT

We provide non-emergency medical transportation (NEMT) services across Ventura County and the surrounding areas.
Booking requests are subject to availability and confirmation.
Clients must provide accurate and complete information when booking services.

PAYMENT & CANCELLATIONS

Payments must be made at the time of service or as per the agreed terms.
Cancellations made within 24 hours of the scheduled transport may incur a cancellation fee.
Be aware of the invoice short and long-distance terms, agreement, and cancellation policy

SAFETY & COMPLIANCE

Our team follows strict safety protocols, ensuring safe and reliable transportation.
We comply with all applicable regulations and guidelines, including HIPAA.

LIMITATIONS OF LIABILITY

We are not liable for delays caused by traffic, weather, or unforeseen circumstances.
MediZoom Transportation LLC will not be responsible for any damages arising from the misuse of our services.
The company is not responsible for any medical emergencies that arise during transport. Passengers who require emergency medical care must arrange for ambulance services.

USER RESPONSIBILITIES

Clients must ensure that any necessary medical equipment or documentation is available during transport.
Any inappropriate behavior or violations of safety protocols may result in the termination of services.
Passengers are expected to behave respectfully and courteously to drivers, staff, and other individuals during transport
Refrain from any illegal activity or disruptive behavior
Comply with any safety instructions given by the driver

We are committed to protecting your personal information as outlined in our Privacy Policy.
MediZoom Transportation, LLC complies with HIPAA.

MODIFICATIONS

We reserve the right to modify these terms at any time. Continued use of our services constitutes acceptance of the revised terms.
